YOUR PRIORITIES LEAD

One family. Many goals. One connected plan.

POH helps families turn the future they envision into practical steps—then connects coaching, information, community, and resources around those goals.

IMPORTANT CLARIFICATION

POH is not primarily a direct financial-assistance program.

The program’s core is coaching, goal setting, resource navigation, community support, and steady progress. Small stipends may sometimes recognize completion of specific program commitments or activities, but they are not the main service.
