THE IDEA AT THE CENTER

Families are experts in their own lives.

POH does not hand families a predetermined definition of success. Families identify their own priorities, then work with staff and one another to build plans, access support, and strengthen the relationships that help progress last. The model is individualized enough to respect each family’s circumstances and connected enough to build a genuine community.

FOUR PARTS, WORKING TOGETHER

How the program is delivered

Individual coaching

Self-directed, individualized support to define goals, create plans, and monitor progress.

Family Saturdays

Monthly sessions strengthen relationships and create a supportive community.

Responsive workshops

Trainings shaped by what families say they need.

Resource connections

Help navigating healthcare, housing, financial, and other services.

THE FAMILY JOURNEY

From an idea to meaningful progress

The path is not always linear. Families can revisit goals, adjust plans, and respond to new opportunities.

01  Listen — Your family defines what success means.

02  Plan — You and your coach build achievable steps.

03  Connect — Navigate services and opportunities.

04  Participate — Family Saturdays add shared learning.

05  Progress — Recognize growth, adjust next steps.

06  Lead — Share knowledge and shape what comes next.

THE ROLE OF A COACH

Someone to help turn goals into next steps

A coach listens first. Together, families and coaches identify priorities, create an action plan, connect with useful services, and revisit progress.
